What to Bring

Because the office is completely digital, there are no forms to fill out before coming. After checking in, you will be asked to spend about 5 minutes at our secure reception area kiosk to fill out brief demographic information and a medical history form.

Bring as many questions as you have in addition to any paperwork and radiographs your dentist may have provided.

  • Thurs. | 10:00am - 6:00pm
  • Fri.     | 10:00am - 5:00pm

For your convenience, we do our best to schedule around school hours so that your children will have to miss as little school as possible. Office hours change in the summer when school is not in session.
